Cialis: All You Need To Know

Cialis, also known as Tadalafil or Adcirca,  is a popular erectile dysfunction drug that has gained notoriety and usage over the last few decades or so. While primarily used for impotence, it’ can also be used for pulmonary arterial hypertension and helps the artery blood vessels to open up . It’s also used for  benign prostate hyperplasia (enlargement of the prostate) , and other prostate and circulation problems.

History of Cialis

Tadalafil was originally developed by the Glaxo Wellcome in 1991, in partnership with ICOS . It was originally developed as a heart medication. Around the same time a competing pharmaceutical company, Pfizer, was developing a similar product known as Sildenafil, more commonly known as Viagra. During clinical tests, they noted that male patients tended to get erections as a side effect of the drug. ICOS later partnered with Eli Lilly and Company to further develop it further. It, along with Viagra was approved by the FDA for customer use in 2003, and was approved for treatment of pulmonary arterial hypertension in 2009.

How Cialis works

Cialis works by blocking a chemical known as PDE5. When a man gets an erection, the blood vessels in his penis dilate, allowing it to fill with blood. During stimulation, Nitric Oxide is released into the genitals. This causes a substance know as cGMP to be released.  PDE5 destroys cGMP, causing arousal to end. Thus, Cialis combats this process. The walls of artery muscles also contain PDE5, which is why it works well for cardiovascular issues in addition.

Dosage

Doses usually come in 5,10 and 20 milligram doses. The pills tend to be yellow in color, and are almost always almond shaped. It is favored by some over Viagra due to it’s longer effectiveness of 17-18 hours. Sildenafil, by comparison, only has a half life of 4 to 5 hours. Contrary to popular belief, Cialis does not give one an “instant erection”. Sexual stimulus must still take place for it to be effective. It’s also not recommended to take more than one dose in a 24 hour period, and for daily use, no more than 2.5 to 5 mg for someone sexually active at least twice a week, unless recommended by a physician or general practitioner .

Side effects

Side effects vary, but the most common seem a prolonged erection and a sudden drop in blood pressure, more commonly known as hypertension. Priapism, an erection that lasts 4 fours or more, is very dangerous and can cause permanent damage to the genitals. They should also not be mixed with nitrate medication, as they can cause heart and chest problems. In some people, it can cause sudden blindness and restrict blood flow to the optic nerve. This is most common in diabetics and people with high cholesterol/heart disease. It’s also not recommended for use by anyone with general heart, kidney, liver or eye problems.

Availability

As mentioned earlier, many prefer Cialis to Viagra due to it’s longer duration of effectiveness. But other factors are also in play as well. Food and duration after meals  tends to effect the usefulness of Viagra, where as with Cialis it does not. Also, Cialis is a once daily pill, which many prefer to constantly re-dosing with Viagra, which is a “take as needed” solution that may not be preferable. One factor in Viagra’s favor however, is it’s less likelihood of negative reactions for men with  kidney or liver dysfunction. Finally, Cialis tends to be cheaper on the whole, starting at about .50 for a 5 mg tablet, all the way up to $ 28 for a 20 mg.

Viagra comparatively starts at for a 50 mg pill, up to for a 100 mg tab.

Where to buy in South Africa?

In South Africa, both Viagra and Cialis require a prescription to purchase. Many other places,  such as neighboring Australia and the  UK offer generic over the counter versions, albeit requiring consultation with a trained pharmacist among other things.  Despite this, many men purchase it online as an alternative. This is highly dangerous, as there is no guarantee where it was made or where it was shipped from, along with being highly illegal to receive.  There are currently no plans to implement an over the counter version of Cialis at this time.
